Although no fields are considered mandatory, we offer the following guidance to help you fill out the form:

The first section of the form is strictly for identification purposes and is straightforward. We wish to have an E-mail address for each person, if possible, as this cuts down on the cost of communicating with one another. Please be careful to enter your correct email address because what you enter is what we receive. We assure you that we do not share email addresses with any other association or agency.
The purpose of the 'Major Health Issues' section is to provide a short, point-form listing of issues that you are currently facing or have faced in the past. By including this section, others may wish to contact you as they may have experience with similar difficulties.
The purpose of the 'Therapies/Programs' section is to obtain a listing of any special therapies or programs with which your child has been involved. The idea here is that perhaps another parent may receive a recommendation that their child become involved with a particular therapy. As such, they may wish to contact you to obtain your viewpoint as to how this program worked for you and how it benefited your child.

And, of course, the Directory works on a reciprocal principle. Not only do you offer the benefit of your experiences to the other members, but they also offer you the benefit of their experiences. Everything you record on the registration form will be included in the directory. The information is kept strictly confidential and is distributed only to the other members of the directory.
